What You’ll Do:
- Provide skilled physical therapy services to patients in a home health setting
- Assess patient needs, develop treatment plans, and implement care strategies
- Educate patients and families on therapy techniques and injury prevention
- Document patient progress using Meditech Home Care EMR system
- Collaborate with healthcare team members to ensure optimal patient outcomes
- Travel within a 50-mile radius to visit patients at their homes
- Participate in a one-week orientation to familiarize yourself with facility protocols and equipment
What You Need:
- Valid Physical Therapist license in Louisiana
- BLS certification
- Minimum of 1 year of acute care or relevant experience
- Strong assessment and interpersonal skills
- Ability to work independently and travel as required
