- Outpatient-only role within a multidisciplinary, value-based care model
- 35 patient-facing hours per week with 5 hours of dedicated admin time
- Panel of 20 - 25 patients per day in a mobile clinic setting
- Collaborate with specialists, care navigators, pharmacists, and social workers
