What You’ll Do
- Conduct patient evaluations through history-taking, systems review, and specialized assessments
- Develop individualized treatment plans with patient-centered goals to reduce pain and restore function
- Administer therapy techniques addressing specific impairments and in line with physician orders
- Monitor and reassess patient progress regularly to refine treatment strategies
- Manage discharge planning from initial visit through ongoing care
- Supervise and delegate tasks to ancillary staff and students per state laws
- Complete accurate documentation within established timeframes
- Educate patients, families, and caregivers to promote understanding and compliance
- Collaborate with healthcare team members to ensure optimal patient outcomes
What You Need
Required:
- Valid Texas Physical Therapy license
- Experience in Long Term Care or similar setting
- Strong assessment and patient management skills
- Excellent communication and teamwork abilities
Preferred:
- Prior experience in a long-term care environment
- Knowledge of current best practices in physical therapy
