- Shared call among all providers; RN call center handles routine calls
- Only urgent matters escalate to the physician, supporting great work-life balance
- Manageable patient census of 10 - 12 patients
- Value-based care model with focus on quality outcomes
- Experience with eClinicalWorks and HEDIS highly valued
- DEA license required
- Board-certified or board-eligible geriatricians encouraged to apply
