Schedule
Day Shift: 7:00 AM-5:00 PM
Call Required
In-House and Beeper / Phone Call
Weekend Call: Approximately 9 Weekends Per Year

Patient Population / Volume
Adult and Geriatric Patients
Approximately 75% Inpatient / 25% Outpatient
Approximately 4-5 Patients Per Day
Level I Trauma Setting

Clinical Responsibilities
Provide Inpatient and Outpatient Cardiac Electrophysiology Care
Perform Electrophysiology Procedures
Perform Cardiac Ablations
Perform Pacemaker Procedures
Perform ICD Procedures
Perform BiV Device Procedures

Requirements
Board Certified in Electrophysiology - Required
Ohio Medical License - Preferred
BLS - Required
ACLS - Required
DEA - Required
Electrophysiology Fellowship - Required
Minimum 1 Year of Work Experience - Required

Submission Requirements
Must Disclose Whether Provider Has Previously Worked Within the Health System
If Previously Employed or Contracted, Must Provide Facility, Last Date Worked, and Agency

Credentialing
Estimated Credentialing Timeframe: Approximately 3 Months With Ohio DEA and Medicare Enrollment in Place
Credentialing May Take Approximately 4 Months Without Existing Ohio DEA and Medicare Enrollment
Temporary Privileges May Be Available"
