• 3-month proctorship required for applicable candidates

    

• MD or DO from an accredited medical school

    

• Completion of an ACGME/AOA-approved 3-year residency in EM, FM, or IM

    

• ECFMG certification required for foreign medical graduates

    

• Active, unrestricted Virginia medical license

    

• Active VA DEA with Virginia address

    

• DEA schedules II, IIN, III, IIIN, IV, and V

    

Accepting:

• ABEM

    

• AOBEM

    

• ABMS-certified Emergency Medicine

    

• ABMS-certified Family Medicine

    

• ABMS-certified Internal Medicine

    

For providers who are not EM board certified or EM trained:

• ACLS

    

• ATLS

    

• PALS

    

• NRP required for non-ABEM providers

    

• MMR and Varicella vaccines or positive immunity titers

    

• TB testing within the past 12 months or QuantiFERON Gold within 3 months

    

• Second TB test required before activation/privileges

    

• Chest X-ray within 3 months if TB history or QuantiFERON is positive

    

• Current-season flu vaccination

    

• Case logs from the previous 2 years

    

• Documentation of specialized training for special procedures, if applicable

    

• Preeclampsia CME required initially and every 2 years for reappointment
