Momentum Health Staffing is seeking a full-time Family Practice Physician to join Community Health and provide comprehensive primary care at clinics in Castleton and Rutland, Vermont.
Opportunity Highlights
Full-time Family Practice Physician opportunity
Practice at established clinics in Castleton and Rutland, Vermont
Provide comprehensive primary care services to patients across the community
Empower patients to take an active role in their health and promote healthy lifestyle behaviors
Provide appropriate patient follow-up and maintain effective communication with patients
Refer patients to specialty care when clinically appropriate
Provide clinical supervision to mid-level providers and nursing staff
Participate in quality assurance and continuous improvement activities
Participate in State and National disease collaborative efforts
Work within a highly developed Level III Medical Home environment
Comprehensive integrated care model including:
Primary Care
Behavioral Health
Care Management
Social Work
Diabetes management and education
Robust integrated behavioral health program with Psychiatrists, Behavioral Health Nurse Practitioners, and Licensed Clinical Social Workers
Fully integrated electronic medical record and practice management system with strong hospital interfaces
Supportive multidisciplinary team focused on quality, accessibility, equity, and exceptional patient care
4-day work week providing an excellent work-life balance
Average on-call responsibility of approximately 3 weeks per year
Qualifications
MD or DO degree from an accredited medical school
Completion of internship and residency in Family Medicine or related primary care specialty
Board Certified in the practicing specialty
Current or eligible for a Vermont medical license
Current CME compliance for licensure and board certification
Strong inpatient skill set
Quality-focused team player with excellent organizational and problem-solving skills
Ability to work effectively as part of a multidisciplinary team
Excellent oral and written communication skills
Commitment to providing exceptional patient care
