Full-time Geriatric Medicine leadership opportunity
Lead the development and implementation of a new geriatric service line
Dual focus on:
Direct patient care
Consultative and educational support for the local provider community
Key responsibilities include program design, clinical care, and oversight of clinical operations
Establish and nurture strong relationships with internal and external healthcare stakeholders
Provide consultative and supportive services to an independent Skilled Nursing Facility (SNF) service line
Community Health provides exclusive medical care to three Level I Skilled Nursing and Subacute Rehabilitation facilities in Rutland
Geriatric and SNF services will operate as distinct and independent service lines
Leadership position reporting directly to the Chief Medical Officer
Services will be provided at a newly constructed facility in Rutland, Vermont
4-day work week offering an excellent work-life balance

Qualifications

Graduate of an accredited medical school
Completion of an Internal Medicine or Family Medicine residency
Board Certified in Internal Medicine or Family Medicine
Geriatric fellowship training and subsequent Board Certification preferred, but not required
Eligible for medical licensure in the State of Vermont
Experience working with geriatric patients
Strong communication and relationship-building skills
Ability to provide clinical leadership and develop a new service line
