Overview:
Our hospice organization in North Chicago is seeking a Registered Nurse (RN) Case Manager to provide comprehensive nursing care to patients with life-limiting illnesses in their homes and community settings. New grads are welcomed to apply for this full-time role within a collaborative interdisciplinary team.

Position Offers:

Schedule: Monday–Friday (Full-time)
Competitive salary: $70,000–80,000 per year (DOE)

Key Responsibilities:

Conduct comprehensive nursing assessments of patients' physical, emotional, and psychosocial status in home and community settings
Develop, implement, and evaluate individualized hospice care plans focused on symptom control, comfort, and quality of life
Provide expert pain and symptom management, including medication administration, monitoring, and timely intervention
Educate patients and families on disease progression, care options, medications, and what to expect at each stage of care
Coordinate care and communicate regularly with physicians, social workers, chaplains, aides, and other team members
Perform home safety assessments and implement interventions to reduce risk and promote comfort and caregiver safety
Document all assessments, interventions, and patient responses accurately and promptly in the EHR
Participate in interdisciplinary team meetings, case conferences, and on-call rotations to support continuous patient care

Qualifications:

Active, current Registered Nurse (RN) licensure in Illinois
Prior experience in hospice case management, including full-cycle patient care, preferred
Certification in Hospice and Palliative Nursing (CHPN), or willingness to pursue, preferred
Valid driver's license and reliable transportation required
