A skilled nursing facility serving Worcester and Milford, Massachusetts, is seeking an experienced MDS Coordinator to join its team. Key Responsibilities
Complete and submit accurate, timely MDS assessments in compliance with CMS/RAI guidelines
Coordinate the interdisciplinary team to ensure assessments, care plans, and documentation are completed on schedule
Develop, review, and update resident care plans based on assessment findings
Monitor documentation to support accurate PDPM classification and reimbursement
Track assessment schedules and submission deadlines
Stay current on federal and state regulatory requirements related to MDS

Qualifications
Minimum of 1 year of MDS experience required
Strong knowledge of the RAI process, PDPM, and CMS regulations
Excellent organizational skills and attention to detail
Ability to collaborate effectively with nursing, therapy, and administrative staff
