Overview:
Our healthcare team in Brooklyn seeks a reliable and compassionate Licensed Practical Nurse (LPN) to provide high-quality resident care and support daily health needs. Our LPNs collaborate with a multidisciplinary team to promote wellness, safety, and continuity of care. This role is suited for professionals committed to patient-centered and culturally sensitive care.

Position Offers:

Full-Time Schedule
Competitive Compensation: $70,720–$79,040 per year (DOE)

Key Responsibilities:

Administer medications and immunizations as prescribed
Monitor resident conditions, record vital signs, and report changes
Provide wound care and support chronic condition management
Assist residents with activities of daily living, including hygiene, mobility, and feeding
Document all care accurately in the electronic health record (EHR)
Educate residents on medications, wellness, and self-care practices
Collaborate with clinical staff to coordinate individualized care plans
Follow infection control protocols and respond to clinical emergencies as needed
Qualifications:

Active and unrestricted LPN license in any U.S. state
Prior LPN experience preferred; background in urgent care, behavioral health, or corrections is a plus
Strong communication and interpersonal skills
Ability to deliver culturally sensitive, patient-centered care
