Perm - Dietitian - Long-Term Care (LTC)/Rehab (Days) – Brattleboro, VT

Pay & Benefits:
- Hourly Pay Rate: $28 - $37/hour (dependent on experience)

What You’ll Do:
- Conduct comprehensive nutrition assessments for residents at nutritional risk
- Develop and implement individualized nutrition care plans
- Evaluate the need for specialized nutrition support and provide appropriate recommendations
- Document assessments accurately in medical records
- Participate in quality improvement and care team meetings
- Support patient and family-centered care initiatives
- Assist in preparing the LTC unit for survey readiness and quality improvement projects
What You Need:
- Valid license as a Registered Dietitian (RD)
- At least 1 year of experience in long-term care or inpatient nutritional care
- Strong knowledge of JACHO, state, and federal regulations
- Excellent communication and documentation skills
- Ability to work collaboratively within a multidisciplinary team
