Clinical Responsibilities
Provide Outpatient Neurology Coverage
Evaluate New Consultations
Manage Established Patients
Perform Botox Injections for Migraines if Credentialed
Perform EMGs
Interpret EEGs
Provide Neurologic Patient Management and Follow-Up Care

Requirements
AANP or ANCC Certification for Nurse Practitioners - Required
NCCPA Certification for Physician Assistants - Required
Arizona License Highly Preferred
ACLS - Highly Preferred and Must Be Willing to Obtain During Credentialing
BLS - Highly Preferred and Must Be Willing to Obtain During Credentialing
DEA - Highly Preferred
Clean Malpractice and Background - Required
Specific Availability From December 7, 2026-February 26, 2027 Must Be Provided at Submission
Number of Shifts Available Per Month for Ongoing Coverage Must Be Provided
Must Indicate Interest in Locum-to-Perm if Applicable

Credentialing
Credentialing Timeframe: Approximately 60-90 Days With a Clean File
Arizona Licensure Required for Credentialing
Credentialing Application Must Be Submitted Within 72 Hours of Receipt
