We are seeking a Physician specializing in Internal Medicine for a 120-day assignment in Missouri. Our client requires a dedicated professional to work Monday through Friday, 8 AM to 5 PM. This role focuses on providing preventative geriatric primary care, managing chronic conditions, and ensuring patients receive comprehensive care to minimize hospital admissions. The ideal candidate will be tech-savvy, organized, and proficient in accurate coding and charting.

Responsibilities and Duties

• Provide comprehensive preventative geriatric primary care to keep elderly patients from being admitted to the hospital or referred out.
• Manage a patient panel with a focus on high-risk (15-20%), emerging risk (10-20%), and low-risk (70-80%) cases.
• Address main cases including hypertension, diabetes, hyperlipidemia, CHF, CAD and other cardiovascular conditions, COPD/asthma, CKD and thyroid disorders, osteoarthritis and chronic pain, dementia/cognitive decline, falls, mobility and osteoporosis, and acute illnesses/infections.
• Conduct preventive care and annual wellness visits.
• Perform medication management and provide general geriatric care.
• Ensure accurate coding and charting.
