Statewide Opportunities

Opportunities are available throughout most counties statewide. Assignments are scheduled based on provider availability and county demand, with opportunities in most areas across the state.

Key Responsibilities

Conduct in-home health and wellness assessments

Complete history and physical evaluations, including vitals and basic screening tasks as required

Review patient medications and dosages during each visit

Complete visit documentation using a provided tablet-based system

Requirements

MD, NP, or PA with active, unrestricted Colorado license

National certification required for NP or PA

NPI required

CAQH profile required

Candidates must have at least two years of clinical experience within the past five years to qualify.

Travel and Lodging

Travel and lodging support may be available based on travel distance and provider availability.
