Our client in Indiana is seeking a dedicated Family Practice Nurse Practitioner for an immediate start on a 120-day assignment. This role involves a Monday through Friday schedule, with hours varying between 8am-5pm on Mondays, Wednesdays, and Fridays, and 10am-7pm on Tuesdays and Thursdays. The selected candidate will manage medications, educate patients on home treatments, refer to specialists, and ensure continuity of care, diligently charting all patient consultations in the EMR system.

Responsibilities and Duties

• Manage patient medications effectively
• Educate patients on home treatment plans and preventative care
• Refer patients to appropriate specialists as needed
• Ensure seamless continuity of care between visits
• Maintain accurate charting and notation for each patient consultation within the EMR
• Participate actively in peer reviews
• Collaborate closely with the Chief Medical Officer and the medical team
• Provide care to a culturally and economically diverse patient population
• Address general family medicine conditions, including colds, coughs, and flu
• Manage chronic diseases such as diabetes and various heart conditions
• Conduct comprehensive physical examinations
• Deliver preventative care services
• Treat both acute and long-term medical conditions
