Pay & Benefits:

What You’ll Do:
- Provide outpatient primary care for adult and geriatric patients
- Manage preventive services and chronic disease care
- Conduct general evaluation and management in clinic and post-acute settings
- Support SNF and assisted living facilities with limited coverage
- Document patient visits accurately using Cerner/Oracle EMR
- Participate in call coverage (Beeper/Phone), approximately 1 call per day
- Collaborate with a dedicated healthcare team to ensure quality patient outcomes
What You Need:
Required:
- Family Medicine Certification/Training
- Active Vermont License (preferred)
- Experience providing outpatient adult and geriatric care
- Ability to manage chronic and preventive care services
Preferred:
- Outpatient primary care experience
- Geriatric patient care experience
- Minimal malpractice history
