- Estimated Credentialing Timeframe: 60-90 days

- Clinic + Beeper Night call
Clinical Details - Clinic-based Practice
- SNF & Assisted Living Coverage (less than 1.0 FTE)
- 100% Outpatient
- Trauma Level Designation: N/A
- Providing outpatient preventive and chronic disease management
- Provide care across clinic and post-acute settings
- General primary care evaluation and management
- Preventive care services
- EMR: Cerner / Oracle
Requirements - Family Medicine Certification/Training Appropriate for NP or PA Practice - [REQUIRED]
- Active Vermont License - [PREFERRED]
- Providers with Lapsed Board Certification - NOT ACCEPTED
- Primary Care Experience - PREFERRED
- Adult Patient Care Experience - REQUIRED
- Geriatric Patient Care Experience - REQUIRED
