What You’ll Do
- Provide comprehensive initial assessments and ongoing patient care in accordance with federal, state, and agency standards
- Develop, implement, and modify individualized care plans
- Conduct patient education for patients, families, and caregivers
- Complete all required documentation, including OASIS data
- Collaborate with healthcare team members to coordinate care and discharge plans
- Promote a safe, clean, and confidential environment
- Supervise CNAs and coordinate care activities
What You Need
Required:
- Active Registered Nurse (RN) license in Illinois
- Minimum 1 year of relevant nursing experience
- Ability to provide patient-centered care and conduct assessments
- Strong communication and teamwork skills
Preferred:
- Experience in home health or similar settings
- Bilingual abilities are a plus
