What You’ll Do
- Provide hospice care visits to patients in their residences
- Collaborate with interdisciplinary teams to develop care plans
- Manage patient symptoms and provide comfort-focused interventions
- Document patient interactions and care delivered accurately
- Educate patients and families about hospice services and support options
- Ensure compliance with all healthcare regulations and standards
What You Need
Required:
- Current Nurse Practitioner (NP) license for Texas
- Experience in hospice, palliative, or related care
- Strong communication and compassionate patient care skills
Preferred:
- Prior experience with home health or hospice care
- Ability to work independently and manage a patient caseload effectively
