Clinical Details

• Work Setting: Outpatient primary care clinic with in-person and virtual visits
• Average Patient Volume: 16 patients per day
• Patient Demographics: Adults 18+, predominantly geriatric Medicare Advantage patients age 65+
• EMR: eClinicalWorks (eCW)
• Responsibilities: Acute care, hospital/ER follow-ups, chronic disease management, preventive care, and Medicare Annual Wellness Visits
  • Evaluate, diagnose, and treat adult and geriatric patients with multiple chronic conditions
  • Prescribe medications, order imaging, review results, and coordinate referrals and follow-up care
• Onsite Support Staff: NP, Medical Assistants, Front Office Staff, Care Coordination Team, Clinic Administration, Associate Medical Director

Requirements:

• Local candidates only
• Board Status: Board Certified or Board Eligible in Family Medicine or Internal Medicine
• License: Active, unrestricted Texas medical license
• Certifications: Current DEA registration, BLS
• Minimum 2 years of post-residency outpatient primary care experience
• Experience treating adult and geriatric patients with multiple chronic conditions
• Experience with EMRs and telehealth visits
• Preferred: Medicare Advantage, value-based care, Medicare Annual Wellness Visits, eClinicalWorks, and geriatrics experience
