Description
HealthSouth's Home Health programs provide skilled nursing and therapeutic services, such as physical, occupational or speech therapy, medical social services, and other home medical arrangements. Our team of Home Health professionals works closely with the patient's physician and family to develop a plan of treatment, tailored to meet the patient's individual healthcare needs. This team approach ensures consistent, coordinated care from professionals who understand the unique needs of homebound patients.
The Home Health LVN/LPN is responsible for providing skilled nursing services to the patients under the direction and supervision of the RN case manager, assessing patients & advising the RN case manager on updates to care plan.
In addition, the position:
Requires effective communication skills for working with patients, families, caregivers, and departmental and interdepartmental relationships.
Requires open and on-going communication with the RN Case Manager to ensure patient needs are met.
Who will you treat? We are an acute rehabilitation hospital setting. Typically our LVN/LPNcan expect to spend an average of 4-6 weeks with our patients assisting with a wide clinical caseload such as: stroke, spinal cord injury, brain injury, various neurological disorders, rheumatoid arthritis, osteoarthritis, amputation, joint replacement, fractures, multiple trauma, cardiac conditions, and pulmonary conditions.
We use evidence-based practice guidelines to treat our patients. We utilize our clinical outcomes to assess and improve our treatment interventions and programs. We participate in a national outcomes database, which provides benchmarks and sophisticated outcome reports to help us continually analyze and improve our patient outcomes.
The Home Health LVN/LPN also has specialized Home Health trainings, ability to meet with patients in-house and be an integral part of the continuum of care.

License or Certification:
Diploma or certificate in LPN or LVN nursing, as applicable within the State.
CPR certification (ACLS preferred)
Valid Drivers License and acceptable driving record
Total Education, Vocational Training and Experience:
