Description
Experienced Licensed Practical Nurses needed for Maricopa County Jail on all shifts. Interested candidates must have at least two years paid experience with their license and willingness to float. The following credentials and medical documents are required:
--Drivers license (Front and Back)
--SS card OR Passport OR Birth certificate (front and back with signature)
--BLS certification - AHA only (front and back with signature) CANNOT BE ONLINE COURSE
--Physician statement (dated within the last year)
--TB test (1 PPD reading dated within the past 12 months) OR Chest X-Ray (within last 5 years)
--MMR records (2 different vaccines OR 1 titer from anytime)
--Varicella record (medical history documentation OR 2 different vaccines OR 1 titer from anytime)
