Standard Qualifications: JOB SUMMARY: The ER case manager will coordinate the care and service delivery of selected patient populations across the continuum of illness; promote effective utilization; monitor health care resources; and assume a leadership role with the interdisciplinary team to achieve optimal outcomes. Physical and psychological factors including the identification of cases with potential for high risk complications and acting as an advocate for an individual's health care needs are essential elements of the position. Timely and appropriate discharges will be accomplished through assessment of the patient's post discharge needs and the provision of services necessary to meet those needs. The ER case manager must be knowledgeable in community resources and alternate care facilities. Interacts with physicians, staff, patient/family and outside agencies. Serves to facilitate admission/discharge process: Initiates conflict resolution and refers these issues to the appropriate manager. Collaborates with Director of Emergency Department, Resource Management and Administration to meet facility goals with regard to appropriate utilization of these resources.
