Standard Qualifications: Manager of Case Management Registered Nurse / RN The Manager Case Management oversees a collaborative process that assesses plans, implements, coordinates, monitors and evaluations health care delivery systems. Based on the needs and values of patients and in collaboration with all service providers, the case manager links patients with appropriate resources throughout the continuum of health care settings to assist with achievement of optimal health. Case management is characterized by advocacy, communication, and resource management and promotes quality and cost-effective interventions and outcomes. Oversight of operations of Case Management department and processesAssists in education of new policy and proceduresIdentifies, trends and addresses utilization/denial issues with Medical Staff and AdministrationPerforms analysis of denial trends and attends DMAT meetingsAssists in UR CommitteeObjectively reviews CM department and documents evaluations within specified time frameLiaison between CM, Medical Staff, Administration and other health care professionalsDevelopment of department-specific performance improvement processMaintains current knowledge of Centers for Medicare and Medicaid Services (CMS), JCAHO regulations relevant to case managementAttends Divisional CM meetingsParticipates in staff meetings, patient care conferences and other committees as requiredParticipates in the orientation of new employees and cross training of other case managersParticipates in mandatory CM and hospital in-services and education programsCollaborates with health care team to achieve optimal patient outcomesDemonstrates understanding of HCA, Medical City Lewisville mission, guiding principles and incorporates them into daily performance Medical City Lewisville is focused on providing high-quality care to our community.
