Position Summary & Responsibilities : The Registered Nurse Case Manager is part of a collaborative process that assesses plans, implements, coordinates, monitors and evaluations health care delivery systems. Based on the needs and values of patients and in collaboration with all service providers, the case manager links patients with appropriate resources throughout the continuum of health care settings to assist with achievement of optimal health. Case management is characterized by advocacy, communication, and resource management and promotes quality and cost-effective interventions and outcomes.
