Standard Qualifications:   POSITON SUMMARY  This position is responsible for the provision of medical social work services, financial counseling, mental health counseling, and related assistance to pre-transplant candidates, transplant recipients and their family members. Provides support and advocacy services and linkage with community resources as needed. Works cooperatively and effectively as member of renal transplant team.   ESSENTIAL JOB RESONSIBILITIES  Performs transplant candidate psychosocial assessments and reassessments on a regular basis in accordance with Council of Nephrology Social Work (CNSW) standards. Collects data, involving the patient, significant others, and health care providers when appropriate. Obtains relevant information on social history and reviews history for medical compliance. Assesses motivation and knowledge of transplantation functioning/coping ability and competency for compliance with treatment plan. Conducts a mental status exam if indicated, using appropriate assessment techniques. Develops a social work plan of care based on the identified needs and risk factors. Applies knowledge of the principles of growth and development over the life span to identify patient and family requirements relative to age-specific needs. Obtains appropriate information on healthcare coverage and assesses a transplant candidate's financial situation for ability to manage the costs of the transplant and follow-up care. Obtains relevant information on financial status of transplant candidate. Counsels transplant candidate on extent of existing coverage and the availability of additional financial resources. Develops a "Financial Plan" at the initial interview, reviews it with and provides a copy to the transplant candidate. Links patient to financial assistance and resources as needed. Assists transplant candidate in planning for post-transplant financial stability and health care coverage. Conducts regular follow-up assessments of transplant recipient's functioning, compliance and needs. Provides interventions as necessary to address any problems which interfere with post-transplant plan of care. Obtains relevant information from transplant recipient and available records. Assesses compliance with post-transplant treatment and reasons for non-compliance. Intervenes as necessary to facilitate full compliance, improve functioning, or provide supportive services. Provides information, support, and counseling to transplant recipient and their families in order to help them cope with psychological, social and financial stresses associated with end-stage renal disease and kidney transplantation. Upon referral, provides crisis intervention services to assist the transplant recipients with coping or obtaining required services. Provides brief individual and family counseling as needed. Assists transplant recipients with seeking alternative problem-solving to foster independence in functioning. Provides crisis intervention services directly or by referral as need is indicated. Provides education to patients and families on the psychosocial aspects of renal failure and kidney transplantation. Provides psycho-educational and supportive groups for patients and families as needed Upon identification of need, advocates for patients in obtaining required services and resources. Refers to other providers as necessary to link with necessary services and resources. Coordinates the delivery of services and resources with the transplant team and other providers.  Also includes other miscellaneous direct and indirect services as needed, including, but not limited to: Provides inpatient discharge planning as necessary. Monitors for quality of care in the provision of social work services. Provides formal training to other staff as need is identified. Completes documentation according to standards. Serves as a member of a team and attends meetings as required. Participates in the planning and operation of the program. Assist and participates in service delivery outcome measurement.
