Standard Qualifications: Parkridge Valley Hospital - Chattanooga, TN Adult Social Services Representative- Cleveland Intensive Outpatient Location Details: Main location will be Cleveland, TN, but will need ability to float to Chattanooga and Jasper facilities when needed. Shift Details: PRN Are you interested in working with individuals that may be struggling with behavioral or substance abuse issues? At our Parkridge Valley Adult campus, we give you the opportunity to make a difference in the lives of others in our inpatient hospital programs. Job Summary: The Social Services Representative is responsible for assessing the psychological, social, cultural and/or financial needs that impact the patient's health and recovery. Responsibilities: Utilizes social services, counseling and clinical knowledge in the assessment of patient needs. Through concurrent case management, patients will be assessed to determine appropriateness of admission, continued stay in the program, as well as appropriate level of care. Responsible and accountable for coordination of services through an interdisciplinary process which provides a clinical and psychosocial approach through the continuum of care and balances individually identified patients and family needs with the efficacious and cost effective utilization of resources. Assesses and reassesses discharge planning needs throughout the course of the program. Demonstrate the knowledge and skills necessary to provide appropriate care for patient populations served in his/her department. Assess patients/families ability to learn and incorporate teaching throughout hospitalization and in preparation for discharge. Deliver compassionate, holistic care, while supporting the rights and dignity of all patients. Collaborates with other members of the health care team to deliver quality care across the continuum Education: Master's Degree in related field. Behavioral Health experience in the care of psychiatric, dually diagnosed, or chemical dependency patients strongly preferred. Parkridge Valley , a two-campus behavioral health facility with Parkridge Health System, has served the Chattanooga area for over 40 years, providing expertise in treating a broad range of mental health and chemical dependency issues for all ages. Parkridge Valley Adult & Senior Campus The Parkridge Valley Adult & Senior Campus focuses on treatment of psychiatric and addiction issues in adults.
