Standard Qualifications: Position Summary: To provide exceptional care by providing education to patients with diabetes and their family/caregivers and to provide staff the education they need to provide outstanding care to their patients with diabetes. The Diabetes Educator, (DE) performs an advanced practice role in the delivery of nursing care of high value to meet the needs of patients with diabetes to maintain optimal health and recover from acute illness. The DE utilizes the nursing process which will include assessment, planning, facilitating treatment, coordination, management, consultation, evaluation, implementation, follow-up and revision of the diabetes teaching care plan. The DE will consult the referring physician and other team members as required and offer recommendations for the improvement of diabetes management. The DE provides patients and family/caregivers instruction on diabetes management in both the individual or group setting as appropriate. Instruction is based on individual patient need. The DE serves as a resource to nursing staff regarding diabetes management. The DE maintains continuous quality improvement of the Diabetes Education program. The DE is also involved in the identification and development of educational programs for nursing staff development. The DE functions autonomously and in collaboration with the health care team in education and consultation.
