Standard Qualifications: Critical Care/Emergency Medicine Clinical Specialist  The Critical Care/Emergency Medicine Clinical Specialist is an experienced practitioner with advanced clinical skills; he or she provides direct patient-specific pharmacotherapy and clinical services to the hospital patients following current, evidence-based guidelines and in conformity of applicable medication management standards. The Critical Care/Emergency Medicine Specialist is the resource for clinical medication information for the Intensive Care Unit, Step Down Unit, and Emergency department. Provides clinical consultation and clarification to practitioners. Suggests appropriate, cost-effective therapeutic alternatives to medical staff as needed.  · Monitors drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies and appropriateness of drug and dose.  · Sustains the hospital formulary, minimizing non-formulary procurements, utilizing therapeutic interchanges and promotes rational drug therapy selection.   Provide double-checks with nursing for high alert medications (i.e. alteplase, heparin) when available.  Work collaboratively with other clinicians and health-care providers to implement and maintain innovative disease management programs and clinical pharmacy services.  Participate in the development of medication use management programs within John Randolph Medical Center including core measures, clinical guidelines, critical pathways, disease management, and drug use programs.  Provide medication history assessment for risk-stratified patients.  Actively participates in codes (i.e. Code Purple, Code Stroke, Code Blue, Code 99, Code Sepsis, Code STEMI) when available, assisting in running the medication portion of the code carts, providing medication related information and recommendations, dosing and preparing of emergency medications for immediate administration.  Participates collegially in the development of new programs, services and practices, in the education activities of the department and the management and administration of the department.  Participates in providing didactic and experiential training in Clinical Pharmacy for doctoral (PharmD) students.  Takes a lead role in working within clinical initiatives, improving patient safety and medication use.  Facilitates or assists in the medication utilization evaluation process for service line.  Documents all clinical activities and interventions accurately and completely.  Supports and participates in the goals of the organization's quality improvement, risk management, and patient safety programs to include but not limited to National Patient Safety Goals, Core Measures, and regulatory compliance and HCAHPS medication-related initiatives.  Performs other related duties as required. Competent to fulfill Clinical Pharmacist job responsibilities when needed.
