Standard Qualifications: The Breast Health Nurse Navigator provides professional guidance in coordination, execution, and evaluation of their healthcare treatment plan. Identifies patients with suspicious findings or new cancer diagnosis and accepts referrals from physicians and medical staff. Provides patient and family education, communicates with all members of the health care team concerning patient and family needs and concerns. Responds quickly and accurately to requests for information regarding the disease process, community resources, expected side effects of treatment and current clinical trials. Is proactive and identifies needs of the patient for all aspects of care, including social, financial and logistical barriers to prompt and effective treatment. Identifies and works to remove organizational barriers to care, improving efficiency, quality, and customer service. Is knowledgeable of current methods of treatment, new pharmaceutical agents, available community resources, insurance, financial assistance, counseling and support. Facilitates cancer support groups as needed. Provides public education as needed. Demonstrates ability to be self motivated with strong and positive interpersonal skills in interacting with all levels of health care providers and patients. Has a thorough working knowledge of the hospital system in coordinating the patient through the healthcare system. Delivers compassionate, holistic care, while supporting the rights and dignity of all patients. Participates in Performance Improvement activities to assure continuous Quality Improvement. Maintains cultural and diversity awareness when dealing with patients, visitors, co-workers and all other customers. Promotes a supportive, team-work oriented environment. Fulfills departmental responsibilities with the long-range plan, mission, vision and values of CMC. Always smile and make eye contact.
