Corpus Christi Medical Center is a 631-bed Health Care System offering a comprehensive scope of inpatient and outpatient medical, surgical and specialty services, including: 24-hour Emergency ServicesBariatric ServicesBehavioral Health ServicesCancer Diagnostics & TreatmentCardiovascular Testing & SurgeryDiagnostic Imaging/MRIBreast Diagnostic CenterInpatient RehabilitationNeonatal Intensive Care Unit Level I, II, IIIMaternal and Neonatal TransportNeurosurgical and Neurological ServicesOrthopedic ServicesOutpatient Cardiac RehabilitationWomen's Health Center  Corpus Christi Medical Center is currently looking for a Manager - Delivery System Reform Incentive Payment Program (DSRIP) Responsibilities: Prepares/revises the individual project plans in accordance with HHSC guidelines and established due datesPrepares/revises any project plan amendments in accordance with HHSC guidelines and established due dates.Works with Division IT to ensure Chronic Disease Registry (CDR) Updates are on track.Exports and prepares CDR reports for Community Health Workers.Participates in the RHP 4 anchor calls and the RHP 4 Regional learning collaborative.Participates in the HHSC webinars; stays up to date with HHSC's website and any changes that could affect the program.Develops and maintains contacts with RHP 4 representatives, Division DSRIP Director, consultants, and other participating providers to ensure effective communication.Participates in committees and conducts meetings sufficient to satisfy HHSC requirements for metric/milestone achievement.Works with facility and external project leaders to ensure project progress and data submission.Tracks progress and maintains documentation sufficient to satisfy HHSC requirements for metric/milestone achievement.Identifies the data/information required for reporting and ensure all data is obtained in accordance with HHSC's xxxxxxxxxxx*pletes the reporting templates in coordination with the CFO, Division Director and consultants within the established deadlines set by HHSC.Identifies any additional resources necessary to achieve the project metrics/milestones.Tracks Corpus Christi Medical Center's progress towards the category 3 and category 4 quality metrics. Ensures timely submission of CCMC Quarterly Indigent Care Report.
