This position will play a key part in continuing our success! The MSW /MSSW Social Worker provides clinical Social Work Services for patients and families to include but not limited to: Conducting Psychosocial Assessments, family therapy, arranges family meetings with the physician, conducts groups, participates in treatment planning, provides individual therapy, abuse and neglect referrals, manages the discharge needs of the patients. Centennial Medical Center is currently looking for a Social Worker. Responsibilities: Provides Social Work services to patients Conducts group therapyProvides individual and family therapy Provides Discharge planning and coordination of care Demonstrates knowledge of human growth and development, assessment, range of treatment appropriate to the ages of patients servedProvides field instruction for MSSW Social Work student interns as necessaryConducts Psychosocial AssessmentsEnsures compliance with ethical Social Work standardsCan perform Duty to Warn in coordination with physicianAttends and is involved with recovery planning and case reviewsCoordinates with Human Resources for patients regarding FMLA or long and short term disabilityProvides contact and written letters for probation and parole officers for patientsCollaborates with physicians regarding patient care Ensures that Abuse and Neglect referrals are made in accordance with State LawsProvides back to work letters for patientsCan assist with audits ,PI projects ,as chair/ members of committees, make assignments, assist with reportsMay work some weekends
