Overland Park Regional Medical Center is currently looking for a Social Worker MSW - PRN. Position Summary : As a member of the health care team, the social worker provides social work services to patients and families, and other individuals significant to the patient in order to facilitate adjustment to the patient's illness and provide for the patients continuing care needs. Services provided include evaluating patients psychosocial needs, discharge planning, providing crisis intervention, referrals to community agencies and arranging post hospital care, counseling patients and families, and leading therapy and support groups. The Social Worker works collaboratively with other members of the health care team and the patient/family to provide the patient with an appropriate care plan during the patient's hospital stay and continuing care and follow-up after the patient leaves the hospital. Services are provided to the patient/family taking into consideration age specific psychosocial, family and financial factors as they affect the patient's adjustment to their illness.
