Palliative/Hospice Coordinator Position Summary: The Hospice and Palliative Care Coordinator leads hospital-based hospice and palliative care initiatives to improve clinical care of patients with advanced, progressive illnesses or at end-of -life and improve the experience of their families. Clarify specifics of role in collaboration with Chief Medical Officer to tailor to unique needs of facility. In doing this, Coordinator supports nursing, medical staff, administration and whole-hospital efforts to advance hospice and palliative care goals.
