Standard Qualifications: Memorial Hospital (an HCA facility) is located less than 20 miles from the beaches of Jacksonville. We are the premiere medical facility in the Jacksonville area featuring a Certified Chest Pain Center, Certified Stroke Center as well as a dedicated Heart Center. Memorial Hospital is also a U.S. News and World Report Best Hospital. Case Manager Services consist of assessment and discharge planning for individuals, couples, families, and groups. Utilization review for the insurance companies. The RN Case Manager also serves as a resource person to the hospital staff and as a member of the interdisciplinary treatment team. Communicates with the physicians, nursing staff and interdisciplinary team. The RN Case Manager is responsible for performing psycho-social assessments and ongoing assessments related to patient condition and/or plan of treatment to identify and document patient care needs. The Case Manager is also responsible for working to support and protect the rights of each patient served. This includes patient/family emotional support, patient/family/guardian planning care, and directing patient care. The position requires intimate knowledge of both department and hospital policies and procedures relevant to Case Management. The Registered Nurse Case Manager uses and maintains department equipment according to department and hospital policies, and follows hospital and department standards pertaining to information management and quality improvement. The RN Case Manager also provides medical information, treatment plans, goals, and discharge planning needs to insurance companies to explain expected length of stay when appropriate. Interpretive skills, frequent interactive and consultative associations, along with management and supervisory skills are inherent in this position. The ability to retrieve, communicate or otherwise present information in a written, auditory, or visual fashion is essential.
