POSITION SUMMARY: Social Worker MSW - Full Time No Weekends The role of the Master's prepared Social Worker is to perform comprehensive psychosocial assessments on hospitalized patients and their families. Based on that assessment, the social worker shall provide emotional support, education and coordinate appropriate resources to promote quality and continuity of care, as well as appropriate utilization of hospital resources. Typical Duties (including but not limited to): The social worker will work in conjunction with nursing, physicians, case management, child life and ancillary hospital departments to assess the acute and post-hospital needs of all adult patients and infants through 17 years of age. The social worker will coordinate resources and the delivery of services, which will allow the patient and family to function at an optimum level upon discharge. The social worker shall demonstrate knowledge of social and physical factors that affect adult and pediatric patients/families and knowledge of community resources to assist them while hospitalized as well as assist the patient/family with discharge from the hospital. The social worker shall demonstrate knowledge of the principles of growth and development over the life span. The social worker shall possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify the needs of the patients and families. The master's prepared social worker will provide consultation to bachelor's prepared social workers. The master's prepared social worker will participate in program development and process improvement and implementation.
