Summary: The Nurse Navigator for MountainView Hospital's Orthopedic and Spine Program facilitates care for patients by serving as a liaison throughout the continuum of care. This is achieved by anticipating and addressing patients' needs before, during, and after their hospitalization, in consultation with the medical team, to include the patients' surgeons and the Orthopedic and Spine Surgery Program Director. Facilitating a team approach that includes surgical nurses, floor nurses, case managers, and physical therapists is key to success of this position. In addition to the day-to-day activities mentioned above, the position requires the Nurse Navigator to conduct data review to identify trends in patient experience and complication rates. Once trends are identified, the Nurse Navigator will assist in implementing strategies to achieve optimal care for future patients. As such, the Nurse Navigator serves as an advocate for both the patients and the program. A majority of the Nurse Navigator's time will be spent providing education to the patients. This shall include developing and presenting a pre-operative class for patients, held often and regularly. The Nurse Navigator is also responsible for extending the teaching and education support to the patient's family, if requested by the patient. Upon discharge, the Nurse Navigator will develop and utilize patient literature that can be used once the patient returns home, to remind them of discharge instructions.
