The Spine Care Navigator will serve as the primary navigator and experience guide for patients within the outpatient or inpatient setting, or, depending on program size, both. The NN is responsible for facilitating the patient experience by guiding patients across the care continuum, including navigation across various providers within a service line or across the treatment pathway, including both surgical and non-surgical treatments. The NN is the primary point of contact with the patient and serves as the intermediary between the patient and the care provider or hospital resource, responsible for patient education, scheduling, care transition, discharge, post treatment follow up, etc. Depending on the size of the program, the NN may also serve as the program leader, responsible for program development, oversight and management
