Portsmouth Regional Hospital is the only Level II Trauma Center on the Seacoast and one of only three in New Hampshire to earn this important distinction.  Position Description Position Title Department  Infection Preventionist / Sepsis Coordinator  Infection Prevention 941 and 917 POSITION SUMMARY: The criteria on this Position Description are intended to describe the general content of and requirements for the performance of this position and are not meant to be an exhaustive statement of duties, responsibilities or requirements. Works in collaboration with the medical and hospital staff to provide evidence based guidelines and protocols to facilitate multidisciplinary identification of and care to the septic patient. Provides ongoing education to hospital staff regarding sepsis management. Provides guidance to physician on both goal-directed care bundles and documentation requirements for Sepsis. Serves as abstractor for Sepsis core measure. Assists and supports an active, effective hospital-wide Infection Prevention Program under the direction of the Infection Prevention Director.  SPECIFIC ELEMENTS & ESSENTIAL FUNCTIONS: Infection Preventionist • Assists the IP Director in managing, evaluating and assuring the effectiveness of the Infection Prevention Program on an ongoing basis  • Maintains a working knowledge of Infection Prevention related standards, guidelines and laws. • Assists in providing Infection Prevention education, orientation, and training programs to the staff in accordance with statutory and regulatory requirements (e.g. new employee orientation,) and hospital policy in the absence of the IP Director. • Performs surveillance related to compliance with hand hygiene, device utilization, and precautions. Tallies, documents and reports data as required • Assesses and collaborates with other members of the healthcare team and coordinates the collection of post discharge data for patients with infectious and/or communicable disease • Collects , tallies and documents post-discharge surgical surveillance data • Reviews microbiology results to differentiate results that need reporting or further review as a potential Healthcare Associated Infection (HAI) • Serves on and actively participates in designated interdisciplinary committees and task forces for accomplishing hospital wide goals when needed in the absence of the IP Director.  • Serves as a liaison with members of the health care team to elicit and resolve potential/existing problems. • Recommends implementation of preventative and/or corrective procedures/processes. • Performs duties in an independent manner. Uses critical thinking skills, and additional resources to identify and solve infection control problems. • Recognizes and seeks assistance, when needed, to problem solve.  • Applies good verbal and nonverbal skills to achieve service goals of customer service and satisfaction.  • Collaborates with all members of the interdisciplinary team to achieve productive movement of the best practices within the organization.  • Maintains active involvement in nursing and hospital committees as necessary.  • Performs Environmental Infection Prevention rounds within all departments and hospital areas on a regular schedule. Shares findings via a standardized report with department directors and at the Infection Prevention Committee meetings.  Sepsis Coordinator • Provides hospital-wide leadership of Sepsis program • Ensure Early Goal Directed Therapy requirements are met. Responsible for outcomes measurement for selected population. • Collects and analyzes data to drive performance. Performs Core Measure abstraction. • Provides direct and timely feedback to providers to assist with documentation, coding, clinical care, and outcomes.  • Facilitates development and refines the sepsis protocol to become the standard of care within the facility, while providing and participating in the quality improvement process.  • Conducts regular Sepsis Program meetings for the evaluation of outcomes, policy and protocol development, review of current best practice through networking and ongoing education, and to provide input into enhancement of current Sepsis Program.  • Coordination of process improvement activities across departments.  • Responsible to track patients across the continuum to assure outcomes.  • Responsible for staff development regarding care of the Sepsis patient and development of related materials.  • Collaborates with Corporate Division staff on implementing Sepsis care initiatives
