For more information, please visit our website at xxxxxxxxxxx* and select Northeast Methodist Hospital under Locations.   General Statement of Duties:  Will work closely and collaboratively with the medical staff, other hospital departments, community agencies and others to provide continuity in care to the patient and their families. Responsible to assess strengths and/or stressors that impact patient functioning in order to identify available resources. In addition will assess the needs of the organization and coordinate process improvement opportunities in areas to include but not limited to physician, patient and employee satisfaction with the Case management services, patient throughput and availability of community resources.  Essentials Job Functions:  1. Evaluates patient's physical, mental, and emotional well-being by interviewing patient, family and significant others when performing psychosocial assessments. Makes recommendations to the health care team regarding psychosocial diagnosis and treatment options when appropriate.  2. Assess the psychological and emotional support needs of adults with illness and their families, on an age appropriate basis. Assume responsibility for counseling and/or coordinating with appropriate team members/caregivers.  3. Collaborates with the health care team to facilitate discharge planning, coordinating patient care conferences as appropriate and ensuring documentation of assessments, interventions & plans are recorded in the medical record to include collaboration with patient/family, staff, community agencies and others as appropriate. Makes referrals to appropriate community agencies, maintains up-to-date information on community resources.  4. Provides short-term crisis intervention counseling and emotional support to patients and their families, with focus on alleviating anxieties/fears relative to their life altering conditions and uncertainties of recovering. Assists patients/families to adjust to hospital routines; understand, accept, and comply with medical recommendations related to their selected interventions. Recognizes the need for long-term intensive counseling services and makes appropriate referrals. 5. Reviews hospital and department data/feedback to identify process improvement and training opportunities to enhance patient throughput and satisfaction with case management services.  6. Coordinates plan to carry out needed training or process changes facilitating physician education, nursing and new case management employee training while serving as knowledge expert on topics to include mental health commitments, ethics, documentation, etc.
