Job Description: Responsible for the delivery of patient care through the nursing process of assessment, diagnosis, planning, implementation and evaluation. Coordinates and provides age-specific care for persons with bowel and urinary diversions, draining wounds, fistulas, pressure and vascular ulcers and incontinence. Educates staff, families and patients and the community about the specialized care of selected disorders of the gastrointestinal, genitourinary and integumentary systems. Collaborates with other professional disciplines to ensure effective and efficient patient care delivery and the achievement of desired patient outcomes. Collects data for care improvement opportunities.
