Standard Qualifications:  Position Overview: The Care Manager's role is to facilitate integrated care for the patients by identifying and coordinating specific aspects of care including transitions from inpatient to outpatient care, home health care services, medical supplies, and respiratory care. The care manager also plays a role in addressing the needs of patients identified of high utilizers of health care resources. In addition the Care Manager participates in quality improvement projects and collaborates with the Care Managers at the other residency sites to share best practices. Essential Duties: Manage transitions of care - Identify new or existing patients pending discharge who may benefit from transition of care services and meet with patients prior to discharge to identify their transition of care needs - Improve patients' health and reduce per capita spending and utilization (unnecessary hospital and ER admissions) by engaging patients in the management of their chronic conditions by providing access to integrated and coordinated care, by generating referrals to appropriate medical and community resources, and by coaching patients to engage in their plans of care and make healthy lifestyle choices - Maintain a spreadsheet to monitor and manage any new or existing transition of care patients - Meet daily with Medical Director to review all TOC patients to ensure appropriate care and to ensure that clinic is obtaining Medicare payment for transition of care services. - Act as the practice's leading source of knowledge on medical neighborhood resources and their policies and business practices.  Manage the care of high utilizers - Work with Medical Director and social worker to fully address the emotional, physical, behavioral, and environmental needs of high utilizer patients and to provide them with the resources they need to overcome barriers to functional independence and avoid frequent hospitalizations or emergency department visits Manage respiratory care - Manage the care of all patients in need of respiratory therapy by ensuring the correct therapy is ordered and by obtaining required documentaion, oxygen saturations, and diagnosis codes - Coordinate patient access to oxygen, nebulizer, trilogy, CPAP, BiPAP, sleep study tests, over night oximetry tests, and 5 minute walk tests - Maintain patients' certification for respiratory therapy. - Teach attending physicians, residents, and MAs about Medicare referral requirements and payment restrictions Manage medical supply orders for patients - Coordinate access of patients to medical supplies - Follow through with patients and suppliers to ensure patient receipt of supplies - Teach attendings and residents the documentation required to ensure Medicare approval and payment of supply orders Manage home health care - In consultation with attendings and residents, manage referrals for skilled home care such as RN, physical therapy, occupational therapy, CNA, speech therapy, and social work services. - Send home care referrals to home care agencies, communicate with home care clinicians regarding patient condition, update and advise physicians on patient condition, manage the certification and recertification of patients' home care, and ensure that the clinic is appropriately billing Medicare for home care management. Educate faculty in the management of transitions of care patients and respiratory patients - Design, produce, and update training manual the transition of care process - Educate all residents and faculty as to transition of care processes and documentation requirements - Design, produce, and update training manual for respiratory therapy. Educate residents and faculty as to documentation required to sustain therapy, including diagnosis codes and oxygen saturations levels Other Functions - Special projects and duties as assigned - Participate in specific quality improvement projects Financial - Process invoices in a timely manner
