Nurse Practitioner NP / Hospice / Inpatient/Outpatient We are seeking a qualified Hospice Nurse Practitioner NP for an immediate opening. Our client offers the following benefits: $87,000 - $93,000 Salary depending on experience 403b PTO based on hours worked and longevity $20k Life Insurance Long Term Disability Short Term Disability (optional, paid by employee) Nurse Practitioner NP / Hospice / Inpatient/Outpatient Job Responsibilities The Hospice Nurse Practitioner NP is responsible for providing the very best dental care for infants, toddlers, children and teens. Additional responsibilities for the Hospice Nurse Practitioner NP include: Monday - Friday, 8am - 5pm Inpatient rounds in the morning, Home visits in afternoon Provide care to patient when a need is identified during a face to face encounter.Gather clinical information to support the diagnosis and prognosis and provide these findings from the face to face encounter to the physician who is considering recertifying the patient. Provide consultation in palliative care, symptom management and supportive care to meet the needs of the patient as requested by the referring physician. Communicate visit findings to designated members of the interdisciplinary team, including the CMO and RN Case Manager. Participate in a weekly oncall rotation in conjunction with Hospice Physicians and other Nurse Practitioners within the department. Job Requirements We are looking for dedicated and professional Hospice Nurse Practitioner NP who is committed to providing exceptional patient care and service. It is also important that you are highly organized and that you demonstrate excellent communication and interpersonal skills.
