UW Health Rehabilitation Hospital is a free-standing, 50-bed acute inpatient rehabilitation facility located on the east side of Madison, Wisconsin. Our rehabilitation hospital opened in September 2015 and offers specialized programs for people who have experienced stroke, brain and spinal-cord injuries, amputations, complex orthopedic injuries and other conditions requiring inpatient rehabilitative services. Our goal is to help patients achieve their highest level of recovery with a goal of returning home as soon as able. The UW Health Rehabilitation Hospital is a partnership between UW Health, Unity Point Meriter and Kindred Healthcare. PPS Coordinator The PPS Coordinator functions under the Director of CQPI. Provides coordination of the timely and accurate completion of the Patient Assessment Instrument for each rehabilitation inpatient. This individual edits the information contained in the PAI for each patient to assure accuracy in the scoring of functional status and reflection of the patient's care needs. In addition, this position may assist in the ongoing monitoring of identified benchmarks for the inpatient population. The Patient Assessment Instrument Coordinator has access to confidential patient information and maintains confidentiality of all information. Qualifications: Qualifications Graduate of an accredited RN, PT, OT or SLP program with current licensure in the state. Current Basic Life Support/CPR card. Three (3) to five (5) years of experience in the care of patients in an acute hospital or rehabilitation setting preferred. #MON-HRS #LI-HRS
