VITALCARE AMERICA (a Kindred Division)  We are seeking a Full Time Case Manager for our Community Hospital in San Bernardino, CA!   POSITION SUMMARY:  The Case Manager is a LicensedNurse or professional in a healthcare-related field; responsible for  program development for specialty subacute I transitional care unit programsin designated hospitalsor facilities through education to the communityincluding healthcare providersand managed care organizations. The Case Manager conducts on-site and off-site physical evaluations or collects clinical data of potential subacute candidates for admission to the specialty program.   RESPONSIBLE TO:  The Case Manager is responsible to the District Directors   POSITIO N RELATIONSHIP:  Will be a liaison to Administration, Subacute Medical Director, Director of Nursing, Clinical Manager, Social Worker, nursing staff, business office, U.R., and admitting staff of host facility. Will coordinate with discharge planners, social workers, physician(s) of area facilities and insurance case managers. Professional presentation and maintains a professional relationship with all coworkers, healthcare professionals and clients, acts as a "Company representative" to our client facility.   PERSONAL:   - Work effectively with others; demonstrate ability to be tactful, discrete, diplomatic, and patient and work on an "honor system"  - Demonstrate leadership, creative ability, and self-motivation  - Possess good judgment, organizational skills, and good time management skills  - ICU/CCU experience and/or ventilator skills/training a plus  - Excellent communications skills  - Computer knowledge/experience preferred  - Requires access to reliable transportation  - Demonstrate professional image and appearance  - Presentation and public speaking skills a plus  - Maintain current California Drivers License  - Maintain current automobile liability insurance at or above the minimum standards  POSITION RESPONSIBILITIES: CLINICAL:  - Conduct comprehensive patient physicalassessment and medicalrecord review within 24 hours of the requestedevaluation  - Provide complete clinical information through comprehensive evaluation form for admission reviewby Medical Director, admittingPhysician and Clinical Manager, to enable theMedical Director/admittingPhysician to make a determination whether he/she will accept thepatient into the program  - Works with the Discharging facility to provide accuratefamily information to social worker  - Provides "excellent"follow-up communication to the facility DCP that requested the evaluation   PROGRAM DEVELEOPMENT:  - Provide Managed Care contacts, DCPs and relatedorganizations with VitalCare Americaand client hospitalservices information  - Conduct in-services to educate the Managed Care Community, DCPs and related organizations about the servicesoffered at the designated facilityand about the admission criteria for the VCA specialty programs  - Attend seminars/conferences that address continuum of care and Long Term Care  - Provide follow-up communication to discharge plannersand VCA client facilities  - Provide documented referral activity on a monthly basis  - Prepare comprehensive and timely weekly management report to Director of Case  Management detailing the week's activity due every Mondayfor the previous week  - Conduct program development activities throughout the Community  - Relieve/assist other Department membersin program development activities as requested      Qualifications:   If you are a current Kindred/RehabCare employee  Click Here .     QUALIFICATIONS:  RNs  - Graduate of an accredited school  - Additional education/work experience in the area of program development preferred  - Current California License  - Five years of progressive nursing in acute care and rehabilitation, Subacute or home health experience  - Prefer Case Management experience  - Broad knowledge of nursing principles, procedures and practice  - General knowledge of Infection Control standards and practice  - Member and participant in professional nursing organizations  L VNs  - Graduate of an accredited school  - Additional education/work experience in the area of program development preferred  - Current California License  - Five years of progressive experience in the healthcare arenas of acute care, rehabilitation, Subacute or home health experience  - Prefer Case Management experience  - General knowledge of infection control standards and practice
