Summary:  Responsible for planning, organizing, developing and directing the overall operation of the Social Services Department in accordance with applicable federal, state, and local guidelines and regulations, and Kindred policies and procedures. Applies professionally accepted, research based practices, such as Person-in-Environment holistic approach. Works cooperatively as part of the management team to develop resident-centered approaches to all center quality of care and quality of life activities. Resident advocacy responsibilities as needed to achieve or maintain a dignified existence for all residents.  Essential Functions:   Performs general management functions such as hiring, disciplining and evaluating employees.   Participates in new employee orientation, including but not limited to education on resident rights, abuse/neglect, behaviors and culture change/resident-centered care.   Arranges/participates in social services staff orientation   Performs skill testing with other members of Social Services Department; delivers or arranges for additional training for social services staff members when needed   Evaluates Quality Indicators/Quality Measures monthly to audit department performance. Participates as a member of the Performance Improvement Committee.   Develops and delivers culturally-sensitive training to all disciplines, both in didactic and experiential format, to meet the psychosocial needs of the residents. Utilizes classroom training and spontaneous "on the floor" training opportunities.   Directs development and maintenance of a comprehensive list of services that are available to residents living at the center and accessible to residents when they discharge into the community (such as dentists and eyeglass repair shops that accept Medicaid). The list is available for use by center staff in obtaining needed services for residents and by residents and families upon discharge.   Participate as members of the Performance Improvement Committee.   Works with the resident, family/significant other and other team members to outline goals of stay at admission, educational materials, access to services, such as Medicaid, the plan to meet those goals and discharge, as appropriate.   Provides venue for family activism through facilitation of the family council process.   Provides educational materials to resident and/or family/significant others related to advance directives; assists them in completion of advance directives as desired by resident and/or family/significant others.   Maintains professional networking relationships in the community with other social service organizations. Examples of such organizations include but are not limited to: local chapters of National Association of Social Workers and local provider groups of social workers in hospitals or care management groups and ombudsmen.   Provides direct psychosocial intervention to residents and residents' families/significant others. Responsible for timely completion of psychosocial assessments, screening instruments, relevant MDS sections and RAPS.   #MON-NCD    Qualifications:   Education:  Master's degree in Social Work or Gerontology or a Bachelor's degree with a major in the Human Service field, preferably social work. To meet the Human Service field degree requirement, the employee must have at least 30 semester hours or 45 quarter hours either in development of human behavior, family intervention techniques, diagnostic measures or therapeutic techniques, such as social work, psychology, sociology, special education, rehabilitation counseling, and gerontology.   Expected to complete 20 CE hours per year, or per state requirements.  Licenses/Certification:  LCSW or LICSW preferred. Licensure or certification as required by state of employment. LSW's must maintain licensure in states that offer bachelor's level licensure.  Experience:  Two years of social work experience in a healthcare setting, one of which is full-time supervised social work experience, working directly with individuals.   Management/administrative experience preferred.   *Kindred Healthcare proudly supports Affirmative Action.
