Description
SUMMARY STATEMENT: The incumbent shall be responsible for providing case coordination and a full range of social work services including assessment, treatment planning and therapeutic interventions in an interdisciplinary environment consistent with the position's qualifications, professional practices and ethical standards. The incumbent shall also demonstrate accountability for and contribution to program development, quality improvement, problem solving, and productivity enhancement in a flexible interdisciplinary fashion. The incumbent shall be accountable for carrying out all responsibilities in accordance with Kindred CORE values. Potential for Role growth in the future. QUALIFICATIONS: The incumbent shall, at a minimum, be a graduate master's degree program and hold applicable and current license/certification to practice. Academy of Certified Social Workers (ACSW) certification is preferred. Education/experience in working with an adult and geriatric population is preferred. Education/experience serving people with physical disabilities is preferred. Computer and current technology competence.
