Description
Position: RN - Case Manager Hospice Location: San Diego, CA Shift: Full TimeJob InfoCompletes an initial assessment of patient and family to determine home care needs.Provides a complete physical assessment and history of current and previous illness(es).Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patientPrepares clinical notes and updates the primary physician when necessary and at least every sixty days.Participates in on-call duties as defined by the on-call policyRequirementsCurrent CA RN license from a National League for Nursing accredited school of nursing CPR Must be licensed driver with reliable transportation and current vehicle insurance Min. of 1 year nursing experience in the area of hospice or home care preferredWork very well with limited supervision For immediate consideration please create a profile and submit your resume through our applicant tracking system.
