Psychiatric Nurse Practitioner:
A community Health Center is seeking an experienced Nurse Practitioner to treat patients at our 6 primary care clinics located throughout Rutland County, Vermont. Prior experience working with geriatric or substance abuse population is preferred.
Join a dynamic, collaborative team of Adult & Child Psychiatrists, Psychiatric Nurse Practitioners, Licensed Social Workers and Licensed Alcohol & Drug Counselors who together provide a plan of care for our patients. This position will involve coordinating plan of care with primary care providers to include psychiatric consults, completing diagnostic evaluations and ongoing medical management services. Nurse Practitioners receive the support of RN Case Managers.
We are a federally Qualified Health Centers and provide a network of Family Practitioners, Psychiatrists, Dentists and Social Workers. We provides short term and ongoing psychiatric services to any medical patient of the organization. Our behavioral health program is one of the major psychiatric services within Rutland County. We utilize an integrated Electronic Medical Record and Practice Management system.
